Some reminders when filling out:
1. Form must be filled out and emailed in as per directions with $400 deposit and Travel Protection Plan if purchasing on registration by credit card. Form can be used for couples traveling and paying together. Once accepted you will have access to your reservation and can pay future payments online. There is no credit card fee.
2. Put your Club Affiliation on it!
3. Under Cabin selection: Single means you are paying a single supplement, Twin means there are two in the room and Share means you are traveling single but willing to be paired with another single of the same gender. All pairings will be made first within CSC singles before opened to being paired with anyone booking the trip.
Registration opens June 1st - form needs to be emailed in to address on form. There are three options for making deposit due at registration ($400pp + Travel Insurance if purchasing):
1 . Credit Card information can be included on the form when sending in
2. Wait until registration accepted and booked. At that time Mayflower will send you a booking # and you can go online to make payments.
3. Call Michele or Casey and give them your CC information over the phone after emailing the form.
